Cancellation Policy

I authorize South Los Angeles Youth Academy to charge the credit card or debit card noted above for payment of the fees, costs, or expenses, which are related to Player fees. I certify that I am the person who is authorized to use this credit card. I acknowledge that if my payment is late, my credit card and or debit card will be charged. Declined cards will be assessed a $35.00 fee. Accepted Payment Methods: • Credit and Debit Cards: Credit Debit Card Authorization Form Attached • Checks: In memo put players name and team • Cash: Cash only accepted at office • Money Order/ Cashier’s Check: In memo put players name and team • ZELLE South Los Angeles Youth Academy
